Phoenix Mountain Post-Acute is a modern, state-of-the-art care center, located in northeast Phoenix, east of AZ-51 and south of E Thunderbird Road. We are just north of the Paradise Valley Mall on N Tatum Blvd behind the Brookdale Assisted Living.
We feature 130 beds and beautiful spacious rooms in either private or semi-private settings. Guests will enjoy well-appointed accommodations, a diverse activities program, nutritious satisfying menus, housekeeping and laundry services, and beautifully landscaped grounds.
